We promise to make every effort to make your first appointment as easy as possible. Below is some basic information you might find helpful.

For your convenience, you can download the patient forms and complete them prior to your first visit. Then, bring the forms with you when you come in for your appointment. If you choose to fill out the patient form at our office, please arrive 10-15 minutes before your appointment to allow enough time to all paperwork completed.

We recommend that patients wear loose, comfortable clothing and appropriate footwear.
insurance information
We accept all major insurances including Medicare and Medicaid plans. To verify we are in-network with your plan or to obtain your physical therapy benefits prior to your appointment, just call either of our offices and a customer service representative will be happy to assist you. Please have your member id, group number, and patient date of birth available.
